This is a directory of reports and official forms for Rogue Valley Unitarian Universalist Fellowship’s Religious Explorations (R.E.) program for children and youth. Parents, we appreciate you filling out the three registration documents below. Hard copies will be available at the Welcome Table on Sundays and from R.E. instructors.

Registration documents (For parents to complete when enrolling children):

Permission to Use Photographs
Medical Information Form

Forms for special events

Permission to Photograph Form (For photographers and event organizers; three impressions per print-out)
Parent/Guardian Consent and Medical Release Form (For field trips, etc.)
Adult Sponsor Responsibilities (For the adult sponsor of youth attending field trips)
